JOIN THE FORUM
MEMBERSHIP REQUIREMENTS:
-Identify as a female orthopedic surgeon.
-Fellowship trained in sports medicine or fellowship trained with a practice focus in sports medicine. One way to demonstrate practice focus is to be a member of AOSSM, AANA, or PRISM
-Board Certified.
-Nominated by a current Forum member, and completed the application process: Fill out application (sent to you by your nominating member).
-Send completed application and current CV to nominating member by August 15.
-Nominating member reviews the application and CV to verify qualifications and forwards to membership chair.
-Application and CV be completed, reviewed and sent no later than September 1 for attendance at the next annual meeting (MLK weekend).
-Applications and CVs reviewed by board at fall meeting.
-Nominating members and candidates are notified of results of the board review by the membership chair following the fall board meeting. Nominating members should then notify their candidates of the board's decision.
-Qualified candidates are invited to the next Forum meeting by the President, and attendance is final requirement of the application process.
-REQUIREMENTS FOR MEMBERSHIP IN GOOD STANDING:
-Pay annual dues.
-Attend a meeting no less than every three years.
-Present at a meeting no less than every five years.
-Members are responsible for registration fees for all meetings.
